Which MC Manual details the ghaunadan?

This is the ghaunadan - a monster that infests Level 8:

Can anyone verify which specific source (MC annual, etc) contains this monster? I need to cite (preferably) it's original source.

I swore there wasnore of an origin story and while looking i saw candlekeep had a 2e conversion or actual entry of the ghauroper

Gotta check out the ghauroper. I remember something about it, but I think it was a 3E monster.
